What are exosomes, what do they do for the skin, how do they differ from PRP, how many sessions, are they safe? Dr. Sinan Akyürek answers the 10 most common exosome questions.

As a physician working in regenerative aesthetics, questions about exosome treatments are among the things my patients are most curious about. In 2026 this treatment has gone mainstream, yet many questions remain. Here are the ones I encounter daily, answered from my own experience and the current literature.

1. What exactly are exosomes?

Exosomes are tiny “messenger packets” that cells release — nanovesicles 30–150 nm in size. They carry growth factors, proteins and miRNAs. Unlike classic PRP, they carry cellular signals directly, stimulating fibroblasts to produce collagen/elastin. Their biggest advantage: they use the body’s own communication language and don’t behave like a foreign substance.

2. What do they do for the skin?

They raise skin quality from within: reducing fine lines, tightening pores, evening out pigmentation, and adding glow and firmness. I see clear results especially in under-eye darkness, dull skin and early ageing signs. In hair loss they support the follicles; thanks to their anti-inflammatory effect they can also help with rosacea or acne marks.

3. How do they differ from PRP or stem cells?

PRP releases growth factors from platelets; stem cells carry living cells. Exosomes are “cell-free” — they carry only the messengers, making them more targeted and lower-risk. Combined with PRP the effect is stronger, but on their own they are also effective.

4. How is it done — does it hurt?

Usually applied topically after microneedling or fractional laser, sometimes by injection. With numbing cream it is nearly painless; the procedure takes about 30–45 minutes.

5. How many sessions, and when are results visible?

For most patients 3–4 sessions (every 4–6 weeks) is ideal. Glow and hydration increase within 1–2 weeks of the first session; full effect appears in 2–3 months as collagen renews. Results vary per person and are maintained with top-up sessions.

6. What are the side effects — is it safe?

Generally very safe and minimally invasive. The most common are mild redness, sensitivity or swelling (1–2 days). Infection risk is low because of sterile protocols. We don’t recommend it during pregnancy/breastfeeding. With high-quality, purified (MSC-derived) exosomes the risk is minimal.

7. Who should avoid it?

We are cautious with active infection, uncontrolled autoimmune disease, a cancer history, or a highly sensitive immune system. Those on blood thinners may bruise more. In younger patients, if unnecessary, I don’t recommend it — I follow a “minimal intervention” principle.

8. Where do exosomes come from, and how is quality judged?

The best are mesenchymal stem cell (MSC) derived. The products I use are certified and purity-tested. Transparency matters: ask about the source and test reports.

9. Are results permanent — is maintenance needed?

Not permanent, but long-lasting: the body increases its own production. Since ageing continues, I recommend 1–2 maintenance sessions per year. Sun protection and a healthy lifestyle extend the effect.

10. Is it worth it?
