Do Fat-Dissolving Injections Make You Lose Weight? What Lemon Bottle Does and Does Not Do

Dr. Sinan Akyürek explains how Lemon Bottle fat-dissolving injections work, where they make sense, how many sessions, who should skip it, why it is not a diet.

Last month a patient walked into my consultation room and asked, before she had even sat down: “Doctor, if I have three sessions of this fat-dissolving injection, how many kilos will I lose?” The question itself was innocent. The problem was who had taught it to her. For a week she had been watching videos where this injection was presented as something close to a liquid diet.

I gave her the answer I will give you here: zero kilos. And that does not mean the treatment is bad. It means the treatment answers a different question than the one she was asking. Let me explain what it is first; then we can talk about which question it actually answers.

Let me clear this up first: this is not a weight-loss method

Losing weight is like reducing the furniture in an entire house. Diet and exercise do exactly that; every fat store in the body shrinks at the same time. Lemon Bottle goes to a single cupboard in that house. It empties that one cupboard and leaves the rest of the house alone. Expecting it to move the number on the scale is like expecting someone who organises a wardrobe to make your house smaller.

So why does that one cupboard need a separate treatment at all? Because some fat deposits are stubborn. You lose weight, your face slims down, your waist loosens, and yet that small fullness under the chin or that soft band across the lower belly stays exactly where it was. Our bodies are quite stingy about this: they release fat from certain areas last, and from some areas hardly at all. When a patient says “I am already slim, but this part will not go,” that is the spot she is pointing at.

Which is why the right candidate is not an overweight person. It is a person whose weight is where it should be and who cannot make peace with one or two areas. I say this sentence in the first minute of every consultation, because if expectations are not set there, disappointment after the third session is almost inevitable.

What is Lemon Bottle, and what is in it?

Lemon Bottle is a lipolysis solution injected with a fine needle into the fat layer under the skin. It belongs to the family people call “fat-dissolving injections,” which we call injection lipolysis in our own language. The solution contains riboflavin, which is vitamin B2, bromelain, an enzyme derived from pineapple, and lecithin.

How does it work? Think of a fat cell as a locked storeroom. Fat is stored inside, and as long as it sits there, the area looks full. The solution aims to open the storeroom door so that what is inside can leave. But pay attention to one detail: the truck that carries that released fat away is your lymphatic system and your liver. In other words, it is your own body. The injection opens the door; your metabolism does the hauling. That is exactly why I ask for plenty of water, regular movement and light eating between sessions. Open the door but never send the truck, and the load simply waits where it is.

To be honest, that is also the most honest part of this treatment. It does not add anything to you. It asks something of your own system. The chemistry is my job; the logistics are yours.

Where does it make sense?

In limited, well-defined areas. Fullness under the chin leads the list; in many of my patients it is a double chin that has run in the family for generations, regardless of weight. The lower belly below the navel, the inner upper arm and the inner thigh are the other areas I see most often. What they share is this: an area roughly the size of a palm, a soft layer of fat you can pinch between your fingers, and skin above it that still has its elasticity.

I also need to remind you that not every fullness is fat. The sagging area under the chin is sometimes not fat at all but loosened skin or a muscle that has lost its tone. I compare it to the canvas of a tent: if the canvas is not taut, adding or removing padding underneath changes nothing. You need to fix the pole. That is precisely why, during the examination, I grip the fat layer between my fingers and check its thickness. If there is nothing to grip, I do not do this injection. That patient may instead head towards skin quality or tightening treatments that day.

For patients with widespread excess fat over large areas, I say it plainly: unfortunately, this injection will not reduce that volume. That belongs either to weight management or to surgery. Doing the right thing rather than the popular thing sometimes means sending a patient to a different door.

How many sessions, and when do results show?

One session does not finish the job. We usually plan two to three sessions, spaced two to four weeks apart. The reason is not that opening the door once is not enough. It is that carrying away the released fat takes time. With each session the area shrinks a little more, and at the next session we see what is left and continue from there.

In most of my patients the first visible difference appears after the second session. The final assessment is done about a month after the planned sessions are complete, by comparing measurements. I always tell my patients the same thing: do not look for the result in the mirror the next morning. What you will see the next morning is swelling, not slimming.

Swelling and recovery: what happens in the first week?

I never like to skip this part, because this is the period when patients panic most. In the three to five days after the treatment there is swelling, tenderness, a stinging sensation and a firmness you can feel with your hand. Bruising can occur. None of this is a sign that something has gone wrong. It is exactly the reaction we expect. The storeroom door has been opened, and the inside has been stirred up.

Swelling peaks in the first two days and then comes down gradually. During this period I ask you to stay away from hot environments, saunas and heavy sport. The firmness resolves within two to four weeks, and as the area softens, the slimming becomes visible. If the treatment is going on a visible area such as under the chin, do not schedule it the day before an important event or meeting. Give yourself a week of margin.

Rarely, the firmness lasts longer, or there is a feeling of unevenness in the surface. We follow that at the check-up; most of it settles on its own. In all honesty, this is not a painless or trace-free treatment. But it is a short and manageable one.

Who should not have it?

We do not perform it during pregnancy or breastfeeding. If there is an active infection or an open wound in the treatment area, we wait. In people with bleeding disorders, uncontrolled diabetes, thyroid disease, or liver or kidney disease, this treatment does not come up for discussion. Because the bromelain in the solution is derived from pineapple, I specifically ask about pineapple allergy; that detail must be discussed in the consultation.

There is also a non-medical kind of unsuitability: the patient with the wrong expectation. Performing this injection on someone who arrives saying “I am going to lose weight with this without dieting” means failing to give them accurate information. If weight is regained, the remaining cells can grow again. The fat that is broken down leaves, but protecting the area is your homework. An injection given to a patient who refuses that homework is not a good treatment. It is a bad sale.

The “it melted in a week” videos on social media

Some of those videos may show a genuine result of a local treatment. Others are lighting, angle and editing. You cannot tell which is which from the screen, and neither can I. What I do know is this: no injection lipolysis settles in a week, because the body doing the hauling does not run on that calendar.

The faster and more effortless a treatment is described to you, the more questions you should ask. The first thing a good physician will tell you is not what the treatment will do. It is what it will not do.
